hOW TO rEFER A CLIENT FOR OUR DELIVERY PROGRAM

The delivery program requires a medical referral (physical and or mental) , that can be from a doctor, nurse, social worker, or case manager. If the client (s) is (are) eligible the delivery may not begin for 4-8 weeks depending on the date of referral.  I would need the following information as well.

    1.    Address

    2.    working phone number

    3.    physical or mental disability

    4.    household members

    5.    dietary restrictions

    6.    prefers coffee or tea    

Unfortunately, no transportation is not an eligible reason.  I wish we had the capacity to serve anyone without transportation, At the present time we are serving 225 households a month in our emergency program and 150 households in our delivery program.  All done by volunteers of which we are at the lowest number in the 8 years I have been the Director.
